General Requirements

A referral letter
Copies of medical records (including complete outpatient records, summaries of each hospitalization, progress notes, and test results)
CDs with imaging and radiology reports
Results and slides of any bone marrow biopsies (one stained bone marrow smear slide and one bone marrow biopsy slide)
For multiple bone marrow biopsies, include the first and the last results as described above

Acute Myeloid Leukemia	Professor Hee-Je Kim	Mandatory: Bone marrow biopsy results, genetic and chromosomal test results Bone marrow slides, general blood tests and progress notes are not required
	Professor Byung-Sik Cho	Bone marrow slides are not needed (but a report on the bone marrow tissue examination is) Prescriptions from other hospitals for current medications are required
	Associate Professor Silvia Park	General Requirements
Multiple Myeloma (Plasma cell disorder)	Professor Chang-Ki Min	General Requirements
	Associate Professor Sung-Soo Park	General Requirements For biopsies other than bone marrow, include the biopsy results and tissue slides (1 stained, 15 unstained)
	Clinical Assistant Professor Jung-Yeon Lee	The patient must be present for the first visit * No proxy consultations Exclude progress notes from medical records
Lymphoma Chronic Lymphocytic Leukemia	Professor Seok-Goo Cho	A referral letter from the hospital where chemotherapy was administered is required The patient must be present for the first visit * No proxy consultations
	Professor Ki-Seong Eom	Include biopsy results and tissue slides from the confirmed lymphoma site (1 stained, 15 unstained) If diagnosed at Yeouido or Eunpyeong St. Mary's, tissue slides are not required
